This pretty interesting - I guess the media has been photoshopping images for years, but the latest one is Time Magazine running a image of Reagan with a photoshopped tear running down his cheek:

They don't explicitly state that the image has been modified, but the credits read: ""Photograph by David Hume Kennerly. Tear by Tim O'Brien." When asked for an official statement, Time responded that it was a "conceptual cover".
I am quite surprised actually how often this sort of thing happens in the media. The next link is actually quite eye opening:

Just count the number of times heads are spliced onto someone else's body!
